Intake forms are provided in a secure electronic format at time of scheduling. The rate for a 50-minute session is $150 for Individuals and couples. Payments are accepted with check or most major credit cards at the time of service.
In an effort to maintain confidentiality, I do not accept insurance. Claims may be filed with an insurance company directly if so desired. A superbill will be provided at the beginning of each month for the previous month's charges.
Any potential reimbursement for counseling charges would be based on your plan's out-of-network mental health benefits and necessary deductible requirements. I recommend calling your insurance company to inquire about potential reimbursements prior to scheduling.
